Office Policies

 

Patients are seen by appointment only.  We believe in the value of your time and will do our best to keep you from having to wait for an extended time period.  Please understand that some visits require more time, emergency situation arise, and operations sometimes take longer than expected.

All new patients are asked to fill out new patient questionnaires.  Please complete these forms to the best of your ability and bring them along with your insurance card(s) to your appointment.

If you have had any recent lab work or x-rays that may pertain to this office visit, please obtain copies of these and bring them to your appointment or call our office prior to your appointment so that these may be obtained.

It is very important that we have a current list of the medications that you are taking.  If you are unable to list them on your paperwork, then bring your medications with you so the physician can review them.

Our office does not write or refill pain medications after hours, on weekends or holidays.  It is the patient’s responsibility to call our office Monday thru Thursday 9 am – 5 pm, or on Fridays 9 am-noon if you anticipate you will need a refill on your medication.  There are times when medication refills cannot be authorized until the next day.

No child under the age of 18 will be seen by the doctor without consent of the responsible parent.

If you find that you are unable to keep your appointment, please notify our office immediately.  After office hours you may leave a message, and we will get back to you as promptly as possible.

Financial Policies

 

After obtaining your insurance card and information, our office will verify your insurance eligibility and benefits of your insurance coverage.  As a courtesy to our patients, we will file your primary insurance claim to help you obtain payment.  All co-payments and deductible are due at the time of service.

Insurance coverage is an agreement between you and your insurance carrier.  You are responsible for the payment of your bill regardless of the status of your insurance claim.

Patients requiring a referral from the primary care physician will not be seen unless this office has a written referral on file prior to your appointment.  Written referrals are the patient’s responsibility to obtain.

For patients without insurance, charges are due at the time of service.

All accounts are to be paid within three months from the date of service.
